PURPOSE:  The goal of The Hand Center is to provide quality patient care.  In keeping with that goal we wish to advise you that you have the right to expect the following:

• PATIENT RIGHTS AND RESPONSIBILITIES
• ADVANCE DIRECTIVES
• GRIEVANCES
• Nondiscriminatory Policy

The Hand Center has agreed to comply with the provisions of the Federal Civil Rights Act of 1964 be denied benefits of, or otherwise be subject to discrimination in the provision of any care or service.
